YOU WILL BE REGISTERED FOR YOUR CLASS WHEN YOUR ENROLLMENT AGREEMENT/RELEASE OF LIABILITY FORM AND PAYMENT HAVE BEEN RECEIVED.  REGISTRATIONS ARE TAKEN ON A “FIRST COME – FIRST SERVED” BASIS.

I have voluntarily enrolled in a dog training class, consisting of eight group lessons at All Bright Dogs, Inc.  I agree that I may be dismissed from any and all classes at any time, with or without cause, and in such event no portion(s) of my prepaid training fees will be refunded.

I am aware that while participating in the class, I and my dog may be exposed to a variety of environmental conditions which include but are not limited to all types of vehicular traffic, interaction with people and other animals, and exposure to adverse weather.

I further acknowledge that obedience training may be a hazardous activity in which damage or injury to dogs and people may occur.

I hereby certify that any dog I may bring to class, to the best of my knowledge, is in good health, free of communicable diseases, and has had all precautionary vaccinations and protection which may be deemed appropriate for the safety and well being of all human and canine participants attending the classes.

I hereby agree to assume full responsibility and/or liability for any and all risks of damage and/or injury to myself, my dog, and any and all other persons and animals attending the classes.

I agree that I, my heirs, assignees and legal representatives will not make claim against, sue, or attach the property of All Bright Dogs, Inc., the employees of All Bright Dogs, Inc., or any person acting on behalf or at the request of All Bright Dogs, Inc., (hereinafter referred to as “agents”), or any other participants in the class, or invitees thereof, for injury or damage to myself, my dog, or my guests, resulting from action or negligence, howsoever caused, by Laurie Albright, agents, said other participants and invitees.  I forever release All Bright Dogs, Inc., agents, and other participants and invitees from any and all liability and/or demands which I, my heirs, assignees and legal representatives may have or hereafter have for injury or damage to myself, my dog, and/or my guests, and assume all risks thereof.

I hereby agree to mediate and/or arbitrate any misunderstanding that may arise pursuant to the terms contained herein.  In any action or proceeding arising out of the Enrollment Agreement/Release of Liability, the prevailing party shall be entitled to reimbursement of all costs and legal fees.

This Enrollment Agreement/Release of Liability supersedes all other agreements, written or oral, previously made between All Bright Dogs, Inc. or agents and myself.

I HAVE READ THIS AGREEMENT CAREFULLY, FULLY UNDERSTAND ITS CONTENTS, AND AM VOLUNTARILY SIGNING BELOW.  I AM AWARE THAT THIS IS A RELEASE OF ALL LIABILITY AND A BINDING CONTRACT.
